NEW GYMNASTS: Registration forms are to be filled in AFTER N&DGC Head Coach has offered participant a place in a class. If you haven’t filled in an “Expression Of Interest” form to apply for a place in a class, please submit the online EOI first. Wait for confirmation from N&DGC’s Head Coach to confirm you have a place in a class before filling in this registration form.
RETURNING GYMNASTS: No need to fill in the EOI unless you have previously given notice of non-return.
COSTS: Yearly registration fee (covers insurance and membership fee with Gymnastics Victoria).
Term fees will be calculated as per class hourly rate.
Fundraising Levy ($10/gymnast or $20/family per term, invoiced with Term Fees)
Invoices will be emailed to the email listed on the registration form.

Fees Payment Policy
1. I understand that gymnastics is a physical sport, so it comes with a certain element of risk. 2. I confirm that the information on this form is true and correct to the best of my knowledge. I know of no reason or fact affecting the Gymnast’s health, which is relevant to doing gymnastics and heavy exercise, which I have not declared. 3. In case of emergency, I hereby give consent to certified N&DGC personnel to administer emergency First Aid to the Gymnast. I understand that if it is deemed necessary by the first aid officer, an ambulance will be called and I will be responsible for any costs incurred. 4. I understand that for insurance purposes registration fees must be paid before the Gymnast is permitted to participate. 5. Term fees are to be paid within 14 days from invoice date as per Fees Policy. Terms fees are calculated on hours nominated regardless of attendance. Where notice is given in writing of prolonged illness and a ‘Doctor’s Certificate’ is provided, the gymnast’s fees may be refunded or credited to the next term. A written request for a refund will need to be completed and lodged with the N&DGC Committee. Absences for any other reason will not result in a refund or credit, however gymnasts may have the ability to make up sessions during the current term, providing they have organised it with the Head Coach. 6. I give permission for my child to be photographed / videoed while participating in any club activities. I understand this includes media outlets and people requested to take photographs by the club. I consent for these photos/videos to be used for publicity if required, including our website and social media. If you have any issue with this point, please see a committee member before signing. 7. I understand a registration record is kept by N&DGC and is available upon request. 8. I understand that if I choose not to provide personal information, N&DGC may not be able to provide the services the Gymnast requires. 9. I have read, understood and will abide by all policies, procedures and Codes of Conduct of the N&DGC which are available on request at the gym and at www.natigym.com.au. I have read and understood the N&DGC rules and agree to its terms, conditions, consents and declarations as required. I confirm that the information provided on this form is complete and correct to the best of my knowledge and I will advise the Club promptly of any changes that may occur. I agree that N&DGC and its Management Committee, coaches, volunteers, officers, leaders, staff and agents shall be released from, and shall not incur any responsibility or liability whatsoever for any accident or injury to the Gymnast or for any damage to or loss of property of the Gymnast. In that regard I agree to indemnify Natimuk & District Gymnastic Club Inc. against any such liability.
In accordance with the Privacy Amendment (Private Sector) Act (2000), the information contained within this form will be used primarily for matters specifically related to participating in gymnastics and/or if a secondary purpose is related to the primary purpose and one could reasonably expect such use or disclosure. To assist in providing our services, the organisations to which we disclose information include: • Outsourced service providers who manage the services we provide to you, including Gymnastics Victoria, Gymnastics Australia, and Insurers. • Our professional advisers, including our accountants, auditors and lawyers. • Government and regulatory authorities and other organisations, as required or authorised by law. We limit the use and disclosure of any personal information provided by us to such organisations for the specific purpose for which we supplied it.
To apply to become a member of the Association, a person must submit a written application to the Committee of N&DGC, stating that you wish to become a member, support the purpose of the Association and agree to be bound by our Incorporated Rules. There will be a minimal application fee and annual subscription. Membership will allow you to be notified of the AGM and any other Special Resolutions, to submit items and vote at our AGM.
